What causes a face to appear primordially male? There are lots of FFS cases off the top of my head where despite the surgeon doing a good job the patient just looks like a male afterwards. I’m not sure it’s midface because there are actual women with long midfaces and they don’t look like men. So what is it?

I would assume something like very strong facial features starting out. Prominent brow bones, strong chins, etc., would likely be harder to hide completely no matter the amount of FFS. I have a very round face with a not very prominent brow bone and weak chin, however, I have a very distinctive nose. The nose is pretty easy to fix though, since it's just cartilage.

but I would say the usual issue is either a broad square chin, as even women with broad jaws usually have soft chins, or a large browbone, which makes the eye area look very masculine and is often difficult to fix without type 3 forehead reduction

Skull size and expanded facial planes.
Think of it like male faces are more "spread out" and female faces are "compact"

Lip lifts are a thing. It's one of the easiest to fix.

They're only a thing if you have little upper lip otherwise you end up looking like a bunny because they can only lift the middle not the edges
