There's this one cosmetic for the medic that is a little napkin that fits into his coat pocket. It's so small. You can paint it, too. It's barely noticeable. I don't think most people would notice it at all under normal screen resolutions. I've got a 1920 x 1080 display but plenty of people play on smaller resolutions so they have even less of a chance of seeing the napkin. At lower fov's you'd have a better view of it but I don't think anyone should be playing with a low fov just to see cosmetics in better detail. Ultimately the cosmetic ends up being just for you.

The strongest counter to an enemy sniper is switching teams, pocketing them with the Vaccinator on a non-bullet resistance, spamming "nice shot" every time they miss, and occasionally doing the schadenfreude after several consecutive missed shots
