The cyborgs are all dying from the aftereffects of the cyborg surgery, as far as I remember they only have like a year or two if they're lucky (Petra aside). Running or blackmailing the agency won't help, because what the agency is currently doing isn't the issue, it's already a done deal. Running would actually make it worse even if they succeeded, because agency has the best treatment to stall death off, it's the best place to be. Trying to blackmail would probably make it worse because they'd almost certainly notice and kill him (like they killed Claes' handler) and then there'd be nobody left there who cared about Triela. I'm pretty sure he explicitly goes through this exact thought process at one point, doesn't he?

And yes she was dying anyway because of the cyborg conversion, but her lifespan was still being actively reduced every time she went on a mission and was injured, as Hirscher observed. Even if he didn't get help for her and just escaped with her to live in the wilderness she would have at least lived a little longer than she did being purposefully exterminated in an impossible mission against well-armed terrorists orchestrated by the prime minster to cover up his own lose ends, as she eventually was.

The Agency converted her to a cyborg without Hirscher's knowledge or consent, and then expected him to go along with it and be a loyal operative just because he had no other choice. But Hirscher never actually had any motivation to care about the Agency or the fight against Padania, he only ever cared about protecting Triela. I thought that was the perfect setup for him to betray the Agency at a critical moment and run off with Triela to try to save her, which would open up a whole new arc which would pit Triela against her fellow cyborgs and test her individual will and loyalty to her handler against her loyalty to the agency and its mission, which would have been really interesting subject matter. But instead Hirscher just blindly goes along with the Agency's mission until the very end even though it completely contradicts his own motivations, for no clear reason except apparently being too impotent to come up with any third alternatives, even though the consequence for failing any such alternative couldn't possibly be any worse than the fate that was guaranteed if he failed to even try. To say the least I found it a disappointing waste of narrative potential. And on an emotional level, if Triela was going to die, he could have at least made her death meaningful. But he couldn't even give her that.
