All I really have fleshed out is monster-eating occult experiments which escaped the CIA and super patriotic ghoul spooks (replacements for the first guys) who are uncontrollable, overzealous, and slowly assassinating the entire US government for not measuring up to standards.

It has a lot of failings, namely being you cannot play it outside of its built in setting and the freeform nature of Madness powers makes some powers or creative players unreasonably powerful and others near useless, plus its unplayable in large groups (it can handle a maximum of three players and that's if you're good at juggling spotlights).That all being said it's a lot of fun. The surreal City is a great backdrop and it's basically impossible to build a character without a good hook. Plus the Madness powers, when they're strong but don't trivialize things are great.
