As part of the pre-trip inspection before departing for their route bus operators must test the automatic ramp. I've never heard of them being prone to failure. Bus drivers would need to exit their seat to secure the wheelchair so it doesn't roll at an inopportune time and injure the user. There are hooks and straps on most buses but the new BRT buses use a Q-STRAINT device that allows the wheelchair user to secure their chair themselves.
