Haha I wasn't quite sure what you were getting at. There are many different levels of sterility, essentially you have sterile -> not sterile/standard background stuff that's normal in the environment -> body substances that are significantly more likely to be dangerous. So we obviously do everything to avoid contact between body substances and us as much as possible. Everything that is exposed to body substances is cleaned or discarded between patients. And then you have just standard environmental stuff that in EMS we don't really worry about, we don't do any huge procedures involving sterile fields. Any small procedures like an IV stick is made sterile with something like an alcohol pad before being done. All of the equipment is sterile out of packaging, and you have to make sure not to touch the site after swabbing. Things like that are low enough risk that a sterile field is not used.

As for extras:
A flashlight comes in handy more than you'd think.
Little power banks will save you from ever worrying a dead phone and can turn you into an instant hero when someone realizes they're on 3% in the middle of the afternoon because they were fucking around on social media.
A spare mag is a nice addition if you can find the space for it, otherwise don't sweat it.
Pepper spray could be a good idea for that grey area where someone is acting super aggressive, but you're unsure if lethal force would be justified (debatable whether justified use is the same as handgun use for self-defense, but you can't get a life sentence with pepper spray if you make the wrong call).
If you've got your gun riding on a cheap belt or cheap holster, replace that shit; you'll be glad you did.

See what people dont realize, is that even if you have a holster with active retention. If someone goes for the gun, you HAVE to act the same weather it's a lvl 1 or lvl 3 retention, you can not risk your life thinking that they won't be a le to get the holster out.
But it can give you a few extra seconds, and a safari land is industry standard right now, so I am dropping the nearly $200 on a midride.

I have gotten rid of the pouch, which was a med pouch for less minor stuff, parents coming up cause their kid scraped their knee.

I have two and they're pretty great

the only issue I have run into is corrosion
My TRACKER got some rust on it even though it's stainless

the internals of the cylinder in that snubby isn't actually stainless even though its the stainless version and i paid more because of it
i only knew this from an unfortunate boating accident where it was submerged in salt water
and was unable to be cleaned for a week

if you're curious, the extractors rod rusted into the cylinder and seized the action
along with other pieces that seized
i had to beat the cylinder with a plastic mallet to open it
then i disassembled, scrubbed, ultrasoniced for an hour, scrubbed, and doused in balistol
did all that twice and then brought it to the range and shot 200rds, all good after that
