Anon how do I achive this in photoshop?
I don't think it is handmade, it is probably just some kind of script/filter combination on a real photo.

I did this in gimp but I'm sure its the same. Indexed colour like >>373290 said. Set dithering to position and chose 4 colours, I only chose 3 but it looks like 4 in your pic. Then you go into the indexed palette and change the colours to match. If you want the jpeg artefacting then you just export as a jpeg and lower the quality to taste.
